During the final part of the Out Of Commission mission in GTA IV, if you're having problems getting Niko onto the helicopter, here's the explanation...

Basically it's a framerate problem. The game is running at a framerate that is too HIGH (yes that's too HIGH). The higher the framerate at the time you're trying to execute the QTE to get Niko onto the helicopter, the faster you have to rapidly tap the jump button. If the game is running at a high framerate, it is not humanly possible to tap the jump button fast enough to succeed at the Quick Time Event. To overcome this problem, you need to lower the framerate at the time of the QTE.

One way to do it is to download and install the free version of FRAPS, the video game recording software. Then set up FRAPs with a hotkey to record and make sure the FPS for 'Movies' is set to 30 FPS or lower. Then start up FRAPS before you launch GTA IV. Then when you get to the part where Niko jumps the bike off the jetty and grabs onto the helicopter, hit your FRAPS recording hotkey to start recording which will then lower the framerate the game is running at thus allowing you to complete the QTE by rapidly tapping the jump button. I tested this with the paid version of FRAPS and this video is proof that it works (I set the FPS to 30).
