Some other rules not mentioned in the title: No lamping in miniboss fights and no damage buff software combo.
Less of a concrete rule, but I also avoided picking up strong weapons too early, like the axe before the first four guardians. The purpose of this is to replicate what a 'blind' player may have at different points.

Doesn't include Hell Temple because I believe Hell Mushussu is not possible to 'no damage' without the lamp. It's a straight up damage race by design with a few random elements, minions, little room for error, and a very large health pool to top it all off. 'THE BOSS' has very similar traits too, though it is likely possible with chakrams, which is just as lame.
