There are different instances where you can trigger this, one on 1/8 where Kasumi reveals herself to be Sumire, 1/9 which is the day on the video and on 2/2 when Maruki visits Leblanc and you talk to Akechi, if you accept his reality in front of him the bad ending gets triggered.
