Nemesis' door opening script is broken in the PC version of the game. Instead of cutscene taking the control away and Nemesis opening the door, he starts sliding across the room towards the door where his AI is reset, until the necessary script is triggered again. He can be downed in this state and takes full damage.
As mentioned, the special death is also triggered by trying to run 2 rooms away from the direction to the Jill room, like I did on accident in my LP: https://youtu.be/C_Pr-k-ODmk?t=20m33s where I learned of this death for the first time 16 years since I first played the game. The door retains the damaged model until the end of the game.
