This was my 4th playthrough and the only time I got the extra boss. Not sure if you need to play on Normal to get it? Or depending on starting character? I made sure to grab all the treasure chests in this run as well as view all Skits that I knew about.
Also, I killed the adds that showed up during the boss battle. Not sure if I had done that in previous playthroughs.
