In a playthrough of Castlevania II: Simon's Quest Randomizer, I had a weird death that caused the game to freak out and disable collision with a set of blocks on the stairs and stretch the staircase another level. Also there was a visual glitch that caused two purple bars to spawn at that point. It's not necessarily repeatable, but something cool to show off.
